 Researchers found that the gene VKORC1 has a key role in predicting how sensitive a patient will be the blood thinning drug in the first week of using the drug &#8212; helping doctors deal with the wide variation in responses<span id="more-53"></span> to the drug.<br />
 Studying nearly 300 patients starting therapy with warfarin, the researchers found that variations in VKORC1 can explain why individual patients need greater or smaller doses of the drug to best benefit from it. </p>

It&#8217;s not a surprising finding, said Dr. Daniel Woo, an associate professor of<span id="more-52"></span> neurology at the University of Cincinnati and co-author of the report in the Sept. 30 issue of Neurology, since it&#8217;s long been established that the risk of death from such a stroke is greater for people taking warfarin.<br />
But there has been a debate about whether the increased risk is due to an increased volume of blood spewed into the brain. The study of 258 people who had brain hemorrhages appears to settle the issue, Woo said: &#8220;We did find a larger volume of hemorrhage in these patients.&#8221;</p>

  Coumadin (trade name warfarin) &#8220;thins&#8221; the blood by interfering with its normal clotting processes. This action is important in patients who have a condition that may predispose to the development of blood clots, such as some forms <span id="more-48"></span>of phlebitis, a heart-rhythm problem called atrial fibrillation, foreign bodies in the bloodstream such as artificial heart valves, and a host of other conditions. In these patients, Coumadin can prevent blood clots from forming, and thus prevent dangerous complications such as strokes. The degree to which Coumadin is affecting clotting is monitored by a blood test called INR (international normalized ratio), generally checked monthly. Changes in the dose of Coumadin, diet, and the action of other drugs that can affect the metabolism of Coumadin can reduce or augment its actions.</p>

<p>  When you say your blood was &#8220;dangerously thin,&#8221; it sounds like something enhanced the effects of Coumadin and your blood became excessively anticoagulated. Think back to whether you added a new medication, changed doses, altered your diet, or did something else that might have affected the Coumadin.     &#8220;Today is a signif<span id="more-44"></span>icant event for those who see the day when medicine is<br />
tailored to the genetic makeup of each one of its users,&#8221; Lesko said at a<br />
news conference.<br />
    &#8220;We are continuing to engage in various projects that will further<br />
evaluate the relationship between genetic variability and treatment outcomes in<br />
patients,&#8221; FDA Deputy Commissioner and Chief Medical Officer Janet<br />
Woodcock, MD, said at the news conference.<br />
    The change in the label for warfarin (originally sold under the brand name<br />
Coumadin) does not require<br />
that patients get genetic tests. But the new label suggests that genetic<br />
testing can help doctors find the correct dosage of the drug.<br />
    Each year, about 2 million Americans start taking warfarin to prevent blood<br />
clots, heart attacks, and strokes. It is notoriously difficult to find the<br />
right warfarin dosage for an individual patient. Factors such as age, diet, use<br />
of other medications, and use of vitamins and supplements affect the<br />
dosage.<br />
    Because of this, warfarin is the second most common drug (after insulin)<br />
that sends patients to the emergency room.<br />
    Researchers have noticed that about a third of patients who start warfarin<br />
need much lower doses than expected. This difference appears to be due to<br />
variations in two genes. One of the genes, VKORC1, affects a patient&#8217;s<br />
sensitivity to warfarin. The other gene, CYP2C9, affects how quickly a person<br />
clears warfarin from the body.<br />
    The new warfarin label suggests that doctors starting a patient on warfarin<br />
might want to have that patient tested for variations in these genes. Most<br />
major laboratories already offer the test. Lesko says the cost of testing<br />
ranges from $125 to $500; a patient would need to be tested only once.<br />
    Patients already taking warfarin probably will not need genetic testing if<br />
they already have found the warfarin dosage that works best for them.</p>
